While fishing in Castout Creek, you will occasionally encounter Marie the Mad Patient. She is similar to the opponents you've encountered in Icelantica ([[Flying Penguin]], [[Rabid Raccoon]], [[Polar Bear]]), however requires a Lv 73 (Lv. 75+ recommended) pole to successfully attack him.  
After making a cast, you may get this message:

Castout Creek is home to the dangerous Marie the Mad Patient...


Success has a chance of obtaining an item from the [[Collectables#Mad Patient|Mad Patient Collection]]. Different options drop different items. Choosing the Defend option will get you the later items in the collection so you can trade down to complete the set.
What do you want to do?
* Stay calm and don't move.
* Take off and run.
* Offer my next few catches. (depends on the Marie the Mad Patient's hunger)
* Attack with your Candy Cane / Medical Mangler Pole! (requires level 53 Candy Cane / Medical Mangler)

The last option changes depending on which pole ([[Candy Cane]] or [[Medical Mangler]]) you are using, and requires purchasing the fourth add-on for that pole.


Her collection set enables access to [[Burnout Bay]] & Sharpening Oil & Acid Eyes
